Your Initial Appointment
Thank you for choosing The Midwives Clinic to provide your prenatal, birth, postpartum and woman’s health care. In order to provide the best and most efficient care:
- Please complete the following forms and bring to your first visit:
- Registration Form
- Authorization to Disclose/Release Protected Health Information
- Authorization to Leave Personal Health Information
- If you are transferring care from another provider, complete & sign the following form and fax to us ASAP at 206-368-6171. We will strive to get your records prior to your initial visit.
- Arrive 10-15 minutes before your scheduled appointment.
- Bring the following items with you:
- Picture Identification (ie drivers’ license)
- Insurance card and co-pay (if applies)
- Completed Authorization to Leave Personal Health Info Form
- Completed Registration Form
- List of medications / vitamins / supplements
